Overview

Vial blend of a GH fragment sold as 176-191, Mod GRF 1-29 and ipamorelin (12 mg: 6/3/3 mg); fat-loss claims rest on rodent data, and no study has tested the mix.

Sold as a 12 mg vial: 6 mg GH fragment, 3 mg Mod GRF 1-29 (CJC-1295 without DAC) and 3 mg ipamorelin. True hGH 176-191 starts with Phe; vendor 'Fragment 176-191' often lists the AOD9604 sequence, which starts with Tyr, so the exact fragment in a vial is uncertain. The one study of hGH 176-191 itself found a brief rise in blood glucose in rats (Ng 1978); related fragments cut fat gain in obese rodents, and six human AOD9604 trials published safety but no efficacy. Mod GRF 1-29 has no study of its own, and ipamorelin has one human PK study and a negative phase 2 ileus trial (Beck 2014). No study has tested any two of these together. FDA lists ipamorelin in category 2 for 503B outsourcing, and CJC-1295 and AOD-9604 in the withdrawn-nomination table of its compounding safety-risk page.

Benefits

Reduced fat gain (rodent, related hGH fragments); GH release (human, ipamorelin IV; inferred for Mod GRF 1-29); human trials of the analog AOD9604 published only safety. No data for the blend

Dosing reference

Research dose
350 to 700 mcg of blend per injection (about 175 to 350 mcg fragment and 90 to 175 mcg each of Mod GRF 1-29 and ipamorelin at the 6/3/3 mg ratio), 1 to 2x daily: vendor/community range, no published human dose of the blend, hGH 176-191 or Mod GRF 1-29
Range
350 to 700 mcg, 1-2x daily
Reconstitution
2 mL bacteriostatic water
Common vial sizes
12 mg

Subcutaneous in community use, often fasted in the morning or at bedtime. The earlier statement that AOD9604 failed against placebo in 12-week trials is not supported by published data: only safety results of the AOD9604 obesity trials were published (Stier 2013). Human ipamorelin doses were IV (0.03 mg/kg twice daily in the ileus trial); no human dose of hGH 176-191 or Mod GRF 1-29 exists.

Mechanism

Proposed, from component studies. The hGH fragment is the C-terminal end of growth hormone; related fragments raised fat oxidation in obese mice without binding the hGH receptor (Heffernan 2001), while hGH 176-191 itself raised blood glucose and insulin briefly in rats (Ng 1978). Mod GRF 1-29 is a tetrasubstituted GHRH(1-29) analog acting on the pituitary GHRH receptor; its activity has not been measured directly. Ipamorelin is a ghrelin-receptor agonist that released GH without raising ACTH or cortisol in swine (Raun 1998). The idea that the fragment adds direct lipolysis to a GH pulse from the other two has not been tested.

Evidence

Preclinical: animal or in vitro data only

  • hGH 176-191 caused a short-lived rise in blood glucose and a more sustained rise in insulin in rats (Ng and Bornstein, 1978)
  • Chronic treatment with the tyrosine analog AOD9604 raised fat oxidation and reduced weight in obese mice without hyperglycaemia (Heffernan et al., 2001)
  • Six placebo-controlled human AOD9604 trials (about 900 adults, oral or IV) found no change in IGF-1 or glucose tolerance; only safety results were published (Stier et al., 2013)
  • IV ipamorelin in 40 healthy men gave a single GH peak at 40 min with a 2 h terminal half-life (Gobburu et al., 1999)
  • D-Ala2 substitution in Mod GRF 1-29 raised the plasma half-time of GHRH(1-29) only from 4.3 to 6.7 min in men (Soule et al., 1994)
  • Human-trial status: no study has tested the blend as sold or any pair within it. Ipamorelin's only RCT, in postoperative ileus, was negative (Beck et al., 2014); AOD9604 human data are for that analog, not hGH 176-191

Side effects

No human safety study of the blend, of hGH 176-191 or of Mod GRF 1-29 has been published. In rats hGH 176-191 briefly raised blood glucose and insulin (Ng 1978). In AOD9604 trials headache and GI effects occurred at placebo-like rates (Stier 2013), and IV ipamorelin was well tolerated in a phase 2 trial (Beck 2014). Commonly reported by users (anecdotal): injection-site irritation, water retention, joint aches, head rush. Ipamorelin is in category 2 of FDA's 503B interim policy (added 2023-09-29), and ipamorelin, CJC-1295 and AOD-9604 are in the withdrawn-nomination table for 503A, as of 2026-04-22: https://www.fda.gov/drugs/human-drug-compounding/certain-bulk-drug-substances-use-compounding-may-present-significant-safety-risks. FDA cites serious adverse events with CJC-1295, including increased heart rate and a systemic vasodilatory reaction.
