Overview

Vial blend of BPC-157 and TB-500 (Ac-LKKTETQ), two peptides with rodent tissue-repair data; the one rat study of the pair found no added benefit over either alone.

A lyophilized mix of BPC-157 and TB-500, sold as 10 mg (5 mg each) or 20 mg (10 mg each) vials. TB-500 is the acetylated fragment Ac-LKKTETQ, residues 17 to 23 of thymosin beta-4, not the full protein (Esposito 2012). Each component has rodent and cell data on its own: BPC-157 for tendon and ligament healing, TB-500 for angiogenesis and dermal repair. The only study of the pair, a 2026 rat Achilles tendon study (n=32), found both peptides improved tendon histology but the combination gave no benefit over either alone (Biçer 2026). No human trial has tested the blend, and neither component has a completed randomized human trial. FDA lists both on its compounding safety-risk page in the withdrawn-nomination table.

Benefits

Tendon, ligament and gut repair (rodent, BPC-157); angiogenesis and wound repair (rodent and cell, TB-500 fragment); no additive tendon benefit from the pair (rat). No human data for the blend

Dosing reference

Research dose
500 to 750 mcg of blend daily (250 to 375 mcg of each peptide at the 1:1 ratio): vendor/community range, no published human dose of the blend or either component
Range
500 to 750 mcg, 1x daily
Reconstitution
2 mL bacteriostatic water
Common vial sizes
10 mg, 20 mg

Subcutaneous in community use; injecting near an injury is community practice, not a published comparison. The loading-then-maintenance schedule has no published basis. At the 1:1 ratio the daily blend dose carries far less TB-500 than the community TB-500 range of 2 to 7.5 mg twice weekly. The rat combination study dosed by weight, intraperitoneally: BPC-157 10 mcg/kg/day and TB-500 60 mcg/kg/day for 4 weeks (Biçer 2026).

Mechanism

Proposed, from component studies only; no study has measured how the two interact. BPC-157 raised VEGFR2 expression and activated VEGFR2-Akt-eNOS signalling in rat ischemic limbs (Hsieh 2017). TB-500 carries the actin-binding motif of thymosin beta-4, which was necessary and sufficient for its angiogenic activity in endothelial cells (Philp 2003). A 2024 study found Ac-LKKTETQ broken down quickly in serum and suggested its metabolite Ac-LKKTE carries the wound-healing activity (Rahaman 2024). In the rat tendon study the authors suggested the lack of an additive effect may reflect shared downstream pathways, which they did not test.

Evidence

Preclinical: animal or in vitro data only

  • Rat Achilles tendon repair (n=32): BPC-157 10 mcg/kg/day and TB-500 60 mcg/kg/day intraperitoneally each improved histology scores, TB-500 also load to failure, and the combination gave no added benefit over either alone (Biçer et al., 2026)
  • BPC-157 at 10 mcg or 10 ng/kg intraperitoneally restored transected rat Achilles tendon integrity and load to failure by day 14 (Staresinic et al., 2003)
  • The seven-residue actin-binding motif in TB-500 matched full thymosin beta-4 for endothelial migration and aortic sprouting at about 50 nM (Philp et al., 2003)
  • BPC-157 had a plasma half-life under 30 min in rats and dogs, with intramuscular bioavailability of 14 to 19% in rats and 45 to 51% in dogs (He et al., 2022)
  • Human-trial status: no study has tested the blend as sold. BPC-157 has only uncontrolled human pilot reports; no trial of Ac-LKKTETQ has been published, and human data on thymosin beta-4 cover the full protein, a different molecule

Side effects

No human safety study of the blend or of TB-500 has been published, and BPC-157 has only uncontrolled pilot reports totalling under 30 people with no adverse effects recorded. FDA states it has identified no human exposure data for the LKKTETQ fragment. Listed on FDA's 503A category 2 (safety concerns) page, withdrawn-nomination table, as of 2026-04-22, for both BPC-157 and TB-500: https://www.fda.gov/drugs/human-drug-compounding/certain-bulk-drug-substances-use-compounding-may-present-significant-safety-risks. Commonly reported by users (anecdotal): injection-site irritation, transient fatigue, head rush. Both components promote angiogenesis in animal models, so use with active or suspected malignancy is a theoretical concern.
